HS - NEW Student Packet

Welcome to Tremont High School!:

This document will help you learn what to do to access the information you will need to register your student at Tremont High School.  We have listed the forms we will need for you to complete and return to us as well as the sections that are read only.   Please read the following check list, complete the required forms, and deliver with your payment to the High School office at 400 W. Pearl St., Tremont, IL.  OR once forms are received and student is enrolled, you have the option to pay fees on-line. Forms to Complete and Return:

  • Student Enrollment Form

    *Information the office needs to have on file for your child

    Please complete and return both pages of this form.   Please note that the primary parent/guardian information refers to where the child resides.  If there is an alternate address for a parent/guardian where the child does not reside, but you wish us to have contact information for, please see the second page.  Please include 3 emergency contact names, (other than parent/guardian). Include their relationship to your child along with phone numbers.

  • Records Request-Release Form

    This form allows us to request records from your previous school.  Please complete and sign, including the special education section if applicable.

  • Program Participation and Consent/Release Form

    Please complete this form and return.  All students must complete the consent/release and drug test portion on the back side of the form as well, whether or not you intend to participate in sports.  IHSA activities, such as speech, scholastic bowl, band, chorus, etc. are also covered by this waiver.  Students participating in sports will also need an up to date sports physical before they are allowed to participate. PLEASE CALL Mrs. Miller (925-2051 ext. 213) if you have questions about sports physicals.

  • Student Medical Information Please complete one of these forms for EACH child. This form includes general medical, PRN, and emergency information.  Also included on this sheet are general health questions. Please complete with yes/no answers and comments as necessary.    Return this sheet.

  •  Tremont CUSD #702 Acceptable Use Policy (Internet agreement)  

    Please read carefully the rules governing use of school and personal electronic devices.  Both student and parent must read and sign the agreement upon issue of student laptop (please read before signing the Program Participation and Consent Release Form)

  • Data Collection Form

    This form is an Illinois State Board of Education requirement.  Please answer as honestly and accurately as possible and return the form to the school.

  • Home Language Survey

    This is also a required form by the ISBE.   Please complete the answers to questions about primary languages used in your home.

  • McKinney Vento Homeless Education Affidavit

    This form is an ISBE requirement.  Complete as honestly and accurately as possible.  If answering no to the primary questions, please include name of student only on returned form.

     

  • Parking Permit Application

    Please fill out this form if you intend to allow your student driver to drive to school and park in the student parking lot. Students must register the license plate(s) of vehicles used to drive to school.  There will be one pass per student for up to 2 vehicles.  Student have pass visible in the vehicle. Specific spaces are not assigned, only permits.  See section regarding fees in these instructions.

    IHSA has made available a hands-only CPR/AED video for parents and staff. Please view on our district website.

*Fees pro-rated by term, i.e, one semester is $62.50

Receipts available upon request.

 

Lunch Money is collected in a box in the High School office. You may place your cash or check (to Tremont High School) in an envelope labeled with your child’s name, grade, and amount you are depositing in the lunch box in the High School office.  There is no cash in the lunch line; please monitor your child’s lunch account balance and deposit money as needed to avoid a negative balance.

 

School Padlocks are required for Hall lockers and PE lockers.

These locks are available for $6.00 each from the HS office.  You may pick up purchased locks at registration or on the first day of school.

 

Parking Fee $50 – due before student drives to school.  . Note: $25 fee for one semester only.  This fee may be sent to or paid in the High School office.  Passes issued when school begins.

 

Technology Fee $30-   Covers student issued laptops and use of our network. (Payable at issue of laptops)

 

*ADDITIONAL FEES FOR STUDENTS ENROLLED IN THE FOLLOWING COURSES:

Driver Education “Behind the Wheel”  Fee -$100 (due now or when class begins)

Spanish Dual Credit  -$50 book fee (due at issue of book- pay to THS)

Other Classroom/Lab fees as assigned by the instructor (due at teacher’s discretion)

 

Extra-Curricular Activity Fee

The following extra-curricular activities will each require a $50 participation fee.  This list includes, but is not limited to:  Football, Volleyball, Golf, Cross Country, Basketball, Wrestling, Baseball, Softball, Track, Scholastic Bowl, Speech, Fishing, Cheerleading, Dance, and any additional individual participants in IHSA (or IESA) activities.  The following extra-curricular activities will each require a $25 participation fee:  Madrigals, Drama, Musicals, Jazz Band.

 Fees must be paid, sports physicals must be up to date, and the consent /release form must be signed by a parent before students are allowed to try out, practice, or compete.  Please write name of student/sport on memo line of check.  (Sports that start before the school year begins need to have fees paid; physicals handed in and consent release signed no later than registration day if not already sent.) Maximum per year per student of $150; maximum per family of $300.

 

Sex Offender Registration

To the parents of Tremont School District 702:

Pursuant to Public Act 94-004 (Sex Offender Registration), legislation requires that principals and or teachers to public or private elementary and secondary schools notify parents that information regarding sex offenders is available to the public.  This information is available at the following web address:
